Clare Suites Memory Care & Enhanced Assisted Living

A supportive and secure living option

As part of the Clement Manor continuum of care, Clare Suites is a Community Based-Residential Facility (CBRF), a supportive and secure living option for elders age 62 and better who can benefit from enhanced assisted living services and/or memory care. Licensed nursing staff collaborate with residents, their care partners, and physicians to create specialized care plans that support medical needs 24/7. Care is provided in an environment that fosters independence, offers choices, and bolsters a sense of purpose. Clare Suites has 45 private suites and one shared suite.

Memory Care and Enhanced Assisted Living Our Approach to Purposeful Living

Yes, we offer specialized activities tailored to the needs and abilities of Memory Care residents, with a focus on cognitive engagement and social interaction.

Q3. How is individualized care provided for each resident?

Each resident receives a personalized care plan developed by our social work team, based on assessments conducted at admission, to ensure their unique needs and preferences are met. Families are involved with monthly care conferences. We provide 24/7 personalized medical care, rehabilitation services, physicals, podiatry, dental and hearing specialists, consultations, medication administration, and specialized therapies tailored to each resident’s needs.

Q4: Do you take Medicaid?

We do accept it once private pay has been exhausted.

 Q5: What accommodations do you offer?

We have private suites with private bathrooms, weekly housekeeping and laundry services, weekly bath, call pendants, wander guards, social work and pastoral care (and Mass) services, wound care and more. All meals included.
